Create a new form

Start a new form

When you need to build a new form, start by accessing the forms builder and log in with your credentials.

  • Click the New form button on the main dashboard.

  • Type the name of your form in the Title field. This name is visible to your company users in Contractbook dashboard and to respondents when they directly access the form through the URL you share with them.

  • Select the language the form will be presented in from the Language dropdown menu (options include English, Danish and Norwegian).

  • Select JSON from the Template type options for web-based document generation.

Set up form structure with question pages

Every form is organized into question pages, which act as sections that group related questions together. Respondents will move through these pages one at a time.

  • Click on the + Questions page at the bottom of the left-hand sidebar of the new form to create a new section for grouped questions.

  • Type a clear name for the section (e.g., "Employee details" or "Job information").

  • Drag and drop pages to reorder them as needed in the left-hand sidebar, or double-click and select the red Remove button to delete a specific question page entirely.

Add and configure questions

When deciding what question types are available, you have 13 distinct options to best fit the information you need to collect from the form’s recipients.

  • Click Add question on the specific question page corresponding to the question’s type.

  • Select one of the following types from the menu:

    • Short text: Single-line text input for names or job titles.

    • Long text: Multi-line text area for descriptions or special notes.

    • Number: Numeric input with optional minimum and maximum values for salaries or quantities.

    • Date: Date picker for start dates or deadlines.

    • Single choice: Radio buttons for selecting only one option (e.g., yes/no questions).

    • Multiple choice: Checkboxes for picking one or more options.

    • Dropdown: Searchable dropdown list for countries or departments.

    • Table: Structured rows and columns for repeated data entries.

    • Upload: File attachment (up to 20 MB) for ID documents or photos.

    • Company search: Search and select a registered company.

    • Paragraph: Display-only text for instructions or disclaimers.

    • Questions group: Container that groups related questions together (e.g., address fields).

    • Calculation: A calculation is a special question type in your forms that automatically computes a numeric result based on specific previous form answers, such as multiplying a monthly rent by 12.

Configure question settings and validation

Each question requires specific settings to ensure you collect the correct corresponding data.

  1. Create tab

    • Type the label the respondent sees in the Title field.

    • Type optional helper text in the Description field to give the respondent extra context of the expected answer.

    • Type ghost text in the Placeholder field to provide an example format.

    • Toggle the Required switch on if the respondent must answer the question before proceeding.

    • Edit the Question ID to create a unique identifier (e.g., "employee_name") used to map answers into a document template.

  2. Options icon under the Create tab

    • Click the settings icon hovered in blue, located next to the Placeholder text field of the question to add Validation settings which ensure respondents enter the correct data format and length for the specific question (e.g., requiring a specific email format, setting future dates only, or enforcing numeric minimums).

  3. Options tab

    • Type in Description of the question to indicate more details to the form’s respondents who will fill out the answer to it.

    • Toggle Hide title under Other options section to hide the question label from the respondent if the context is already obvious.

    • Use the Tooltip section of the Options tab to add YouTube or any other external link as a Tooltip with custom title and description for the question you are setting up.

Apply conditional logic

To understand conditional logic in forms, you can use it to show or hide questions based on the respondent’s input to other questions they have answered previously in the form’s sequence of questions.

  • Click the Logic tab under the specific question in the form.

  • Set up rules such as "Show this question if" a previous answer equals a certain value.

  • Combine multiple rules using AND (all must be true) or OR (any one must be true).

  • For repeating sections, use a Questions group with a loop condition to repeat the block based on a numeric field value.

Configure the landing screen and general form settings

The landing screen is the welcome page respondents will see before they start filling out the form as soon as they open the form’s direct link.

  • Click on the Start page section of the left-hand sidebar under the form’s Content to customize the welcome message.

  • Type a headline and introduction in the Title and Description fields.

  • Set up a Success message including Title and Message which will appear after the respondent provides all questions to the form and clicks on the final "Submit" button.

  • Upload an Thumbnail photo (minimum 100x100 px) in the Thumbnail photo section.

  • Paste a YouTube link in the Video field to display an introductory video.

  • Navigate to Form settings at the top of the form you work on, to customize your form's URL by changing the Alias field to something descriptive.

  • Select your Visibility setting:

    • Public: Anyone with the link can open and fill out the form.

    • Private: Only users granted access through assigned categories can see the form.

  • Upload any attachments to the form. You can upload up to 50 attachments in total. Maximum file size is 19MB and the formats you can attach are .pdf, .doc, .docx, .jpg, .jpeg, .png, .txt file types

  • Enable the Legal check option to add a required legal check to the form.

  • Enable the Notifications option to send an email notification to the respondent, immediately when they begin completing the form.
